UNOLS-MATE MARINE Technical Internship Placement and Evaluation Forms
Congratulations on receiving an internship position!
Your internship has the potential to be a fantastic life-changing learning experience for you. In order to ensure the best experience possible (and to ensure that you will receive your stipend!), please read all of the information below to help you prepare!
To be completed BEFORE the internship:
___STEP 2: Work with the host organization personnel to complete all the required pre-boarding requirements (the Program Manager will email your flight reservation after obtaining your departure/return airport(s), photo ID information, and flight preferences).
___STEP 3: Apply for the Transportation Worker Identification Credential (TWIC Card):
There is a required $125.25 fee that you must pay upfront when you apply. For reimbursement, email a copy of your receipt and forward the TWIC card confirmation email to the Program Manager.
___STEP 4: Complete the UNOLS-MATE Emergency Contact and Compliance Form, and email to the Program Manager.
___STEP 5: Watch the MERAS Shipboard Civility videos Modules I & II, and review the information in Module III for the specific host organization you will intern with. Read the accompanying Discussion Guide for facilitating communications and preparedness amongst shipboard personnel.
___STEP 6: Complete the pre-internship portion of the Marine Technician Skills Evaluation and email to the Program Manager:
Complete the Pre-Internship Skill Level column by circling the number you feel best describes your current level in Sections II-VII. You can leave everything else blank for now. At the end of your internship, you will fill out the rest of the form, have your mentor review and sign, and then send the completed form to the Program Manager.
___STEP 7: Complete the pre-internship online survey: UNOLS-MATE Pre-Internship Survey.
___STEP 8: Save and print backup copies of the Internship Agreement and Work Plan, and the
Marine Technician Skills Evaluation (with pre-internship skill level completed). You will ask your
host supervisor or mentor to review and sign these forms during your internship.
___STEP 9: Watch the UNOLS Ship Safety Orientation Video and write down any questions you have to discuss during a pre-internship zoom meeting with UNOLS Crewing Manager Meegan Corcoran.
___STEP 10: Participate in pre-internship zoom meetings with the host mentor(s) and the Program Manager to discuss preparations before traveling, what to expect during the internship, and to ask questions. Carefully review all information provided by the host organization to prepare for the internship, and review packing suggestions with the Program Manager before traveling.
___STEP 11: Post a blog entry BLOG LOGIN to introduce yourself and include a photo (limit the size; no stock photos). You will receive a separate email with login information. Review the attached document with instructions. Visit www.marinetech.org/internship-blog to make sure you like the way it looks and share it with your friends and family! This will allow you to address any technical issues in advance.
___STEP 12: Please initial the media release statement below, and sign this form at the bottom to acknowledge that you have received these instructions with accompanying documents and agree to complete all requirements. Email the signed form to the Program Manager before the internship.
To be completed DURING your internship:

___Take 3-5 PHOTOS or videos of yourself during your internship, but DON’T use electronic
devices for personal purposes during work hours without checking with your mentor.
Make sure your mentor reviews and signs the following two forms before demobilization:
1. ___INTERNSHIP AGREEMENT AND WORK PLAN (complete and sign during the first 2 weeks of
your internship with your mentor’s assistance and email to the Program Manager)
2. ___MARINE TECHNICIAN SKILLS EVALUATION (finish completing the form at the end of your
internship before demobilization and ask your mentor to review and sign)
___Post a WEEKLY BLOG entry BLOG LOGIN.
___Participate in weekly check-in meetings with the Program Manager, and one additional
check-in meeting with a Marine Technician that will be introduced by the Program Manager.
Submit the following items within TWO weeks after your internship:
___Complete the FIVE-YEAR CAREER PLAN assignment, email it to the Program Manager with a copy of your updated RESUME, and engage with professional network via LinkedIn.
___Email 2-3 of your best PHOTOS taken during the internship to the Program Manager (please include your initials in the media release statement below):
_______I give permission to the UNOLS Office at the University of Washington to use photos or video for program sponsored or related publications, advertisements, or media presentations used in promotion of the UNOLS-MATE At-Sea Technical Internship Program. I also understand that no royalty, fee, or other compensation shall become payable to me for such use.
___Write a REFLECTION ESSAY (2-3 pages) and email it to the Program Manager:
The reflection essay should summarize what you learned and experienced during the internship, what feedback you received from your mentor(s), and what you will take away from this experience.
___Make sure you have emailed the completed and signed INTERNSHIP AGREEMENT AND WORK PLAN and the MARINE TECHNICIAN SKILLS EVALUATION to the Program Manager.
___Complete the Post-Internship Evaluation online survey: UNOLS-MATE Post-Internship Evaluation.
___Complete an informational interview with UNOLS Crewing Manager Meegan Corcoran to discuss professional career development.
___Complete a post-internship zoom meeting with the Program Manager to verify all requirements have been completed and receipts for reimbursement submitted.
___ Optional: when you have the opportunity, use the PowerPoint template to present/share your experiences as a UNOLS-MATE Intern with other audiences (e.g., a course assignment in school, a presentation at a local school or community event, etc.).
